Transfering Medical Records - New Patients
If you are considering transferring to our practice, there a few steps you need to take first.
- Please check with our office to verify that we currently accept new patients with your insurance.
- Once we have verified that we accept your child’s insurance, all medical records from your child’s/children’s previous physician's office will need to be transferred to our office. Our physicians will then review your child's/children's medical history prior to seeing you for the first time in order to provide better and more comprehensive care.
- Once you have confirmed that we received your child’s/children’s medical records, please give our office 1-2 weeks to review the records before we can schedule any appointments. If you need an urgent appointment for an illness, we will expedite the process in order to see you as soon as possible
